School Forms

As part of our commitment to Green practices and office efficiency, we have carefully considered which forms can be viewed or completed online (saving over 5000 custom-printed pages and another 2,000 copied pages!). In addition, new and continuing families can view personalized registration data safely over the Internet and only change or update as needed.

ONLINE REGISTRATION FORMS - Open June 6th through July 6th, 2010

Classroom assignments and the summer mailing will be sent to families who have successfully completed their registration online. If you did not a get the chance to complete the form while it was unline, please contact the office to arrange to update your form. Thanks. (Updated June 29, 2010.)

Note: When accessing Prospect Sierra School's online forms you will be asked for a unique username and password for your family. This is different from the general username and password used to access the private pages on the school website. Details can be found in the email sent to returning and new families in June. Please call the office if you did not receive your information.

My Forms

The information you provide will be moved electronically into your record at school. Even if there are no changes, you must log in to inform us via an online checkbox that you have reviewed and approve the current data.

If you aren't able to submit your forms during the four week period available, you will be responsible for coming to school to complete the forms.

Process:

• When you click the MY FORMS link (above), You will be greeted with a Log On screen. 
• Use your personal login and password from the mailing sent in May.
• Follow the instructions online.
• Inactivity may trigger a disconnect, so submit often to 'save' your work!
• When you finish each section and press the Submit button, you will receive a confirmation.
• Scroll all the way through, and re-check for errors or omissions.
• Some fields will be required! Use provided buttons to return to re-edit if needed.
• You may log on multiple times during the 3 week registration window.

If you have trouble completing the forms in the four weeks available, open hours at the Avis campus Tech Lab will be available:

Monday through Friday 9am until 3pm
with extended hours on Wednesday from 7:30am until 6:00pm

No computer? No confidence? No problem!

We recognize that some families may not have Internet access at home or work, or may need assistance with the process. The Office Staff or Tech Staff will be able to answer questions or assist you. Anyone is welcome to use the Avis Tech Lab to complete the online forms. Call the Tech Lab at 527-4714 ext. 287 to arrange a drop-in time convenient for you, or to request assistance.

What forms can I complete online?

• Parent and Student Information, including contact, emergency, carpool and medical forms
• Volunteer Form for each adult
• Diversity/Service Learning Resource Survey
• Links to the web to sign up for or edit Escrip and SchoolPop information
• Link to Grandparent Contact and other downloadable forms

You should have the following information to complete the online forms:

• The Online Forms! page from your May packet. It has your personal login and password. (If you lose this page, the school office has a record of your information.)
• Address, Phone, Contact information for you and for other authorized adults with permission to pick up your student in case of emergency or disaster.
• Your out-of-town emergency contact phone
• Medical and Dental information (doctors, insurance)
• Automobile License and California Drivers License

The Parental Consent for Medical/Dental Treatment and Authorization Form (from your mailed packet, or print a copy from this page) must be signed and returned to the office to finalize registration.

Also, be sure to check the box indicating that you have reviewed and approve the information provided.

Mac or PC?

• No problem. This is a web based form. Different browsers (Explorer, Firefox, Safari, Netscape) should work fine.

Office or Home?

• In a couple of cases, people have told us that the forms did not work from computers at work, but worked from home without incident. This is apparently due to firewall restrictions at some businesses. Try at home, or come in to school to use our computers in the Avis Tech Lab.
